Job summary

St. Joseph's Center is seeking a Clinical Care Coordinator to lead nursing services in our ICF programs in Scranton, PA. The role provides on-site clinical oversight, supervises nursing staff, evaluates residents, and ensures person-centered, high-quality care.

The candidate will support regulatory compliance, quality initiatives, and interdisciplinary collaboration, with 2+ years of supervisory experience and 3–5 years clinical practice in long-term care or IDD settings.

Qualifications

• Must have a degree from an accredited program recognized by the State Board of Medicine.
• Must hold a valid Pennsylvania license as a Nurse Practitioner.
• Minimum 2 years of supervisory experience; 3–5 years of clinical experience.
• Experience in long-term care, ICF/ID, or with Intellectual and Developmental Disabilities preferred.
• Regulatory knowledge and ability to support survey readiness.

Responsibilities

• Provide oversight, supervision, and education to nursing staff including evaluating competencies and training on disease management, infection prevention, medication safety, and wound care.
• Conduct routine medical evaluations, provide emergency care as needed, and manage medication orders within licensed scope.
• Ensure CMS and PA ICF/ID regulatory compliance, audit nursing documentation, and support quality improvement initiatives.
• Participate in committees and policy development with the interdisciplinary team to support person-centered care.
• Assist with recruitment, supervise student nurses, and support implementation of EHR and eMAR systems.
