Your Health Organization in Murrells Inlet, SC is hiring a Clinical Coordinator to support clinical needs, manage patient charts, and schedule visits. This full-time role runs 8:00 AM–5:00 PM, Monday–Friday, onsite at the Murrells Inlet clinic, with emphasis on smooth patient care coordination and team collaboration.

The ideal candidate is a CMA or licensed nurse (LPN/RN) licensed in the state, with CPR certification, strong communication, and computer skills to log care management activities

Qualifications

• Must be CMA or licensed nurse and licensed in the state of employment.
• CPR certification required.
• Strong written and verbal communication skills.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure documentation, test results, diagnoses and relevant information are accessible for the provider.
• Coordinate patient appointments with the care team.
• Log care management activities and maintain confidentiality.
