Qualifications

• BBS registration is required for ASW/AMFT/APCC or LCSW/LMFT/LPCC with 1 year of social service experience.
• Fitness for travel and valid driver’s license with vehicle insurance; DMV printout may be required.
• Extensive local travel is required; vehicle reimbursement provided and clients may be transported in the caregiver’s vehicle.

Responsibilities

• Deliver individual, family, group counseling, rehabilitation, and case management for assigned caseload.
• Utilize evidence-based practices to achieve high-quality outcomes.
• Assess risk status and intervene appropriately.
• Create person-centered service plans to support recovery and goals.
• Engage in outreach, report disengagement per protocols.
• Coordinate assessments, referrals, planning, and benefits support.
• Document data and charting according to policy; participate in supervision and training.
• Perform other duties as assigned.
