Job summary

Berks Counseling Center is seeking a Clinical Care Coordinator to manage care coordination services for clients requiring enhanced coordination. The role involves coordinating care plans, referrals, and scheduling to ensure timely access to services, while collaborating with a multi-disciplinary team and supervisor.

You will support data collection, CQI activities, and compliance with applicable standards, with emphasis on accurate EHR documentation and patient-facing services.

Qualifications

• Associate's degree plus at least 1 year of experience in health or behavioral healthcare setting.
• Ability to work 40 hours per week within agency hours of operation (8am–8pm Mon–Thu, 8am–4pm Fri with possible Saturday hours).

Summary of job:

As a Clinical Care coordinator, you will facilitate clinical care coordination services for clients in need of enhanced care coordination. Ensure that clients receive timely and efficient support services through a multi-disciplinary care plan including referrals to both internal and external resources. Activities include data collection and reporting, surveying, overseeing the coordination of care across programs, CQI activities, participating in huddles, and scheduling and assuring patient access to care. The coordinator will work closely with his/her direct supervisor and collaboratively with other department supervisors within the agency.

• Provides outreach and a high level of customer service to referrals sources and collaborative partners.
• Provides direct administrative assistance to departments noted above.
• Participates in overall compliance with all CCBCH, insurance, licensing and other standards and relevant laws/regulations that govern clinical care.
• Supports internal operations within and across programs.
• Assists with the accuracy, timeliness, and compliance of information contained in the electronic health record (EHR).

Requirements

To qualify for the position, you will need...

• Associate's degree plus at least one (1) year of experience working in a health or behavioral healthcare setting or in a similar capacity.
• Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable.
