Job summary

Berks Counseling Center is seeking a Clinical Care Coordinator to facilitate care coordination services for clients in need of enhanced care coordination. You will ensure timely support through a multi-disciplinary care plan, coordinating referrals to internal and external resources, and maintaining data accuracy in the EHR.

Qualifications

• Associate's degree plus at least one year of experience working in a health or behavioral healthcare setting or in a similar capacity.
• Ability to work 40 hours per week within agency hours of operation (8am-8pm Mon-Thurs, 8am-4pm Fri, with Saturday hours when applicable).

Responsibilities

• Facilitate clinical care coordination services for clients in need of enhanced care coordination.
