Community Care Network is seeking a Community Clinician to provide assessment, treatment planning, and therapeutic services to adults in home and community settings in Rutland, VT. The emphasis is on older adults facing transportation or mobility barriers, with collaboration with the Council on Aging and a clinical team.

The role offers independence with support, flexible supervision, and a dedicated office space for care coordination and documentation.

Qualifications

• Masters Degree in Social Work or closely related field.
• Licensed preferred; License Eligible (Rostered as a Non-Licensed Psychotherapist) in Vermont.
• Knowledge of mental health, substance use, and co-occurring disorders.
• Ability to work with older adults in community settings.
• Strong clinical assessment and crisis intervention skills.

Responsibilities

• Provide evidence-based psychotherapy in home and community settings.
• Conduct intake assessments, evaluations, and treatment planning.
• Adhere to admission, continuation, and discharge criteria.
• Complete clinical documentation and data collection per policy and state/federal requirements.
• Coordinate follow-up care and referrals to support continuity of care.
• Deliver services in preferred setting, primarily home visits and community locations.
• Collaborate with internal departments and community partners.
• Participate in supervision, meetings, and ongoing professional development.

About the Role:

The Community Clinician provides assessment, treatment planning, and therapeutic services to adult clients in their homes or community settings, with a primary focus on older adults experiencing transportation or mobility barriers. This role helps ensure individuals have access to high-quality mental health and substance use services where they are most comfortable. Working closely with the Council on Aging and an interdisciplinary clinical team, the Community Clinician enjoys the flexibility and independence of community-based work whilebenefitingfrom a collaborative team environment, supportive supervision, and dedicated office space for documentation and care coordination.

Responsibilities:

• Provide evidence-based, individualized psychotherapy in accordance with best practices.
• Conduct intake assessments, evaluations, diagnostic assessments, treatment planning, and referrals as clinicallyindicated.
• Adhere to program philosophy and established admission, continued stay, and discharge criteria.
• Complete clinical documentation, recordkeeping, and data collectionin accordance withagency policy and the requirements of state, federal, payer, andaccreditingbodies.
• Complete documentationin a timely manner, including treatment plans, progress notes, timesheets, managed care authorizations, discharge summaries, and other required records.
• Coordinate follow-up care, aftercare planning, referrals, and discharge services to support continuity of care.
• Facilitate client access to community resources andadditionalsupports.
• Provide targeted, short‑term case management services as clinicallyappropriate.
• Deliver services in the client's preferred setting, primarily through home visits and community‑based locations.
• Collaborate effectively with internal departments and external community partners, including the Southwestern Vermont Council on Aging, to coordinate client care.
• Participate in regular clinical supervision, consultation, team meetings, department meetings, and agency meetings; seek guidance and support as needed.
• Maintain professional standards of conduct, ethics, confidentiality, and compliance with agency policies and procedures.
• Provide education and training to staff in areas of expertise as requested.
• Participate in community outreach and educational activities as requested by the supervisor.
• Perform other duties as assigned.

Qualifications:

• Masters Degree in Social Work, Clinical Mental Health Counseling, Counseling, Psychology, or a closely related field required.
• Licensedpreferred;License Eligible (Rostered as a Non-Licensed Psychotherapist) in the State of Vermontrequired.
• Knowledge of the assessment and treatment of mental health, substance use, and co-occurring disorders.
• Knowledge of and sensitivity to the unique behavioral health needs of older adults and geriatric populations.
• Ability to provide recovery-oriented, person-centered counseling and support to clients and their families in community-based settings.
• Ability toestablishtherapeutic relationships with clients from diverse backgrounds while maintainingappropriate professionalboundaries.
• Strong clinical assessment, treatment planning, and crisis intervention skills.
• Excellent written and verbal communication skills, includingtimelyandaccurateclinical documentation.
• Strong organizational and time management skills with the ability to prioritize competing responsibilities and manage an independent caseload.
• Ability to work both independently and collaboratively as part of an interdisciplinary team.
• Ability to provide high-quality, client-centered services in a fast-paced environment with limited resources.
• Proficiencywith electronic health records and standard computer applications.
• Must have a valid driver''s license, and reliabletransportation with personal automobile liabilityinsurance coverage at or above the levels mandated by the State of Vermont.
• *Successful completion of a criminal history background check afterhire.
• CompetitivePay Range - $65,000.00-$71,000.00(compensation is typically dependent upon experience)
