Job summary

Lamoille Health Partners in Morrisville, Vermont, is seeking a Medication-Assisted Treatment (MAT) Counselor to join our integrated Community Health Team. You will guide individuals through MAT, provide counseling, and help remove barriers to recovery in a supportive primary care setting.

• Active, unencumbered license in Vermont in appropriate professional scope.
• Experience in addiction treatment and SUD recovery support.
• Strong communication and motivational interviewing skills.

Responsibilities

• Provide individual counseling and crisis intervention for MAT patients.
• Conduct comprehensive psychosocial assessments and motivational interviewing.
• Coordinate care with primary care and behavioral health teams.
• Navigate resources, housing, peer support, and financial assistance for patients.
• Participate in team huddles, quality improvement, and data tracking.

Direct Counseling & Support: Provide individual counseling, crisis intervention, and relapse prevention strategies tailored to patients undergoing medication‑assisted treatment.

Whole-Person Assessment: Utilize motivational interviewing and comprehensive psychosocial evaluations to identify barriers to care, social determinants of health, and individual recovery goals.

Collaborative Care Coordination: Partner seamlessly with our medical and behavioral health teams to manage patient registries, track progress, and ensure smooth transitions across levels of care.

Resource Navigation: Connect patients and their families with essential community services, housing resources, peer support networks, and financial assistance programs.

Quality & Continuous Improvement: Participate in team huddles, quality improvement initiatives, and data tracking to ensure our MAT program continues to meet the highest standards of care.

Who You Are

Licensed & Qualified: You hold an active, unencumbered professional license in the State of Vermont (e.g., LCMHC, LICSW, or LDAC/CADC equivalent) within your professional scope of practice.

Experienced in Addiction Treatment: You have a strong foundation in substance use disorder (SUD) treatment, harm reduction principles, and trauma‑informed care.

A Compassionate Communicator: You excel at building trusting, non‑judgmental relationships and utilize motivational interviewing with ease.

A Collaborative Problem Solver: You thrive in a fast‑paced, multidisciplinary team environment and know how to advocate effectively for your patients.
