Job summary

Valley Health System is seeking a licensed social worker to engage patients and families in identifying psycho-social needs, develop interventions, and coordinate safe care plans. The role includes educating, counseling, and referring as indicated, while collaborating with a multidisciplinary team and community resources to support PFCC information sharing.

The ideal candidate holds a Master’s in Social Work and a New Jersey social work license, with communication and organizational skills

Position Summary

To educate, counsel, and refer as indicated. To plan and communicate effectively with multi-disciplinary team and, community resources and support the Information Sharing Core Element of PFCC.

Education

Master's degree in social work from an accredited program.

Experience

Previous social work experience preferred.

Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations. Ability to use effective analytical and critical thinking skills to problem-solve.
