Acentra Health is seeking a Clinical Assessor, Mid to join our team. The role conducts needs-based eligibility determinations for Medicaid-funded personal care services in home or community settings and assesses level of care to support home-and-community-based living.

Applicants must hold a Registered Nurse or LCSW license, with a minimum of two years' home care experience. Travel is required, and the position emphasizes privacy, dignity, and timely assessments across assigned locations.

Qualifications

• Registered Nurse or LCSW license by the applicable state.
• Minimum of two years' experience in a home care setting preferred.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Ensure PCS are provided on a needs basis based on beneficiary's unmet needs.
• Maintain privacy and dignity of recipients during assessments.
• Coordinate with family members and informal caregivers during assessments.
• Assist beneficiaries in selecting PCS providers and review service plans.
