Job summary

Acentra Health seeks a Clinical Assessor, Mid to join our growing team. The role conducts needs-based eligibility determinations for Medicaid-funded personal care services provided at home or in adult care or supervised living facilities, and assesses level of care to help individuals remain in home- and community-based settings.

This position requires travel to various locations and may involve lifting up to 50 pounds. RN or LCSW licensure and at least two years in home care are required.

Qualifications

• RN or LCSW license in the applicable state.
• Minimum of two years' experience in a home care setting.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved standardized assessment tool(s).
• Ensure PCS are provided on a needs basis based on unmet need and conditions.
• Ensure privacy and dignity of individuals receiving assessment is maintained.
• Conduct assessments within established timeframes for various processes (new, expedited, annual, etc.).
• Interview family members and informal caregivers during assessments.
• Provide guidance to beneficiaries to select PCS providers.
• Conduct service plan reviews as needed.
• Submit completed assessments using state-approved interface.
• Participate in mediation and appeal processes.
• Respond to state inquiries regarding assessments conducted.
• Attend staff meetings and conduct case consultations/peer reviews.
• Provide assessments for initial eligibility determinations for 1915(c) HCBS programs.
• Read, understand, and adhere to HIPAA and related policies.
