Job summary

Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. The purpose of this position is to complete needs-based eligibility determinations for Medicaid-funded personal care services in home or in adult care or supervised living settings, and to assess level of care to keep individuals in home-based environments.

This role requires a valid driver’s license and travel to various locations, with driving up to 8 hours per day and as many as 120 miles per day.

Qualifications

• RN licensure in NC (or compact).
• Valid driver’s license required and travel to various locations.
• Proficient with Microsoft Office Suite.
• Detail-oriented with strong organizational skills.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Ensure PCS are provided on a needs basis based on unmet needs.
• Maintain privacy and dignity of individuals during assessments.
• Complete initial eligibility determinations for 1915(c) HCBS programs.
• Read and adhere to HIPAA and Privacy rules.
• Participate in reviews, mediation, and state inquiries.
