Acentra Health is seeking a Clinical Assessor, Mid to join our growing team. The role involves completing needs-based eligibility determinations for Medicaid-funded personal care services in home or community-based settings, and assessing level of care to help clients stay in or return to appropriate environments.

The position requires active RN or LCSW licensure, travel to multiple locations, and the ability to lift up to 50 pounds.

Qualifications

• RN or LCSW license in the applicable state.
• Minimum of two years' experience in a home care setting.

Responsibilities

• Conduct eligibility assessments for PCS using state-approved tools.
• Assess level of care to support home/community-based services.
• Maintain privacy and dignity of clients.
• Coordinate interviews with family and caregivers.
• Assist beneficiaries in selecting PCS providers.
• Submit assessments via state-approved interfaces.
