Job summary

Acentra Health is seeking a Clinical Assessor, Mid, to join our growing team. The role focuses on completing needs-based eligibility determinations for Medicaid-funded personal care services in home or community settings and assessing level of care to support home-based living.

The position requires a valid license (RN or LCSW) and prior home care experience. You will collaborate with families, maintain privacy standards, and participate in reviews and state-mandated processes.

Qualifications

• Valid RN or LCSW license by the applicable state.
• Minimum of two years' experience in a home care setting preferred.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Determine level of care to allow home/community placement.
• Ensure privacy and dignity of individuals assessed.
• Coordinate with families and informal caregivers during assessments.
• Assist in selecting PCS providers and in service plan reviews.
• Submit assessments and participate in mediation/appeals processes.
