Job summary

Matter of Care, part of the CenterLight Health System family, seeks a Clinical Assessment Manager to conduct comprehensive in-home assessments, identify care needs, and link older adults with needed services to enhance health and independence.

You will work with interdisciplinary teams, document findings in EMR, and contribute to quality improvement while supporting a patient-centered approach to care in the community.

Qualifications

• Current, active, unrestricted New York State Registered Nurse license.
• Minimum of 5 years of clinical nursing experience.
• Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related healthcare settings.
• Ability to travel throughout service areas.
• Strong clinical judgment, communication, and documentation skills.

Responsibilities

• Conduct in-home clinical assessments utilizing the UAS-NY.
• Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
• Perform reassessments and identify changes in condition, functional status, diagnoses, and medication regimens.
• Develop proposed service plans and recommendations to support health and independence.
• Provide hands-on services in participants’ homes as needed, including administering medications and wound care.
• Collaborate with Enrollment, Intake, Care Delivery, and IDT teams.
• Document clinical findings accurately within electronic medical records.
• Participate in quality improvement initiatives and serve as a clinical resource during assessments.
