Job summary

UK St. Claire is seeking a pharmacist to provide comprehensive pharmaceutical care, review medication orders, monitor regimens, and participate in patient care rounds. Collaboration with physicians, nurses, and interdisciplinary teams ensures safe, effective, and cost-conscious therapy.

Education for patients and staff, and support for clinical research and quality improvement, are key aspects. Education includes a Bachelor's or PharmD from ACPE-accredited school.

Qualifications

• Bachelor's Degree or PharmD Degree from ACPE-accredited school.
• 0-2 years of experience in pharmacy operations, or a relevant area with knowledge of TJC, USP 797, 795 and 800, and federal and state regulations An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week.
• Certifications & Licensures Ohio Pharmacist license obtained within 90 days of hire if applicable for work location. Active Licensed Pharmacist in applicable states, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) within 90 days of hire

Responsibilities

• Participates in medication reconciliation to ensure accurate home medication lists.
• Reviews and verifies medication orders for appropriateness, safety, and accuracy.
• Collaborates with providers and multidisciplinary teams for comprehensive medication management and access.
• Educates patients, caregivers, and providers on medication therapy.
• Compounds, dispenses, and supervises the preparation of medications, ensuring quality and compliance.
• Maintains inventory and records for controlled substances and medications.
• Participates in quality improvement, patient safety, and cost management initiatives.
• Provides staff development, preceptorship, and professional enrichment for pharmacy personnel.
• Performs other duties as assigned.
