Qualifications

• Current California Pharmacist license required.
• Inpatient clinical pharmacist experience; PGY1 residency mandatory.
• ACLS or PhART required.
• BCPS preferred; PGY2 residency preferred.

Responsibilities

• Maintain a safe, clean working environment and adhere to infection control requirements.
• Provide cost-effective pharmaceutical care to patients on targeted antimicrobial therapy.
• Monitor and adjust antimicrobial regimens in collaboration with Infectious Disease physicians.
• Communicate medication order changes and document updates in patient records.
• Conduct pharmacokinetic and clinical consultations as needed.
• Educate medical and pharmacy staff about antimicrobials and formulary use.
• Support pharmacy operations and participate in quality improvement initiatives.
