Job summary

Fairview Health Services in Maplewood, MN, is seeking an In Clinic Visit Facilitator to join the Specialty Care team. This 1.0 FTE, benefits-eligible position supports in-person visits, prepares patients, gathers medical information, and helps manage exam rooms and scheduling to optimize the patient experience.

The role requires 1 year of customer service experience and comfort with technology; preferred candidates may include current nursing/medical student or related health program experience.

Qualifications

• 1 year of customer service experience.
• Proficient with technology (computer hardware and programs, smart devices such as phones or tablets).

Responsibilities

• Prepare patients for appointments and collect/verify medical records ahead of visits.
• Assist in rooming patients and document pertinent information and vitals.
• Prepare exam rooms and stock supplies.
• Facilitate patient-provider connections and support a positive care experience.
• Assist with scheduling and coordinating follow-up appointments as needed.
• Communicate effectively with patients, providers and health professionals in person and via telehealth.
