Physiofit, a brand partner of Upstream Rehabilitation, seeks a Patient Care Coordinator to join our team in West Lafayette, LA. This role supports clinic growth through efficient practice management and patient intake, collaborating with the Clinic Director to uphold Upstream Rehabilitation's mission, vision, and values.

The PCC-I coordinates patient scheduling, referrals, deposits, and non-clinical documentation, while ensuring a welcoming, high-quality patient experience across front-office

Qualifications

• High School Diploma or equivalent is required.
• Excellent communication with Business Office and Field leadership.
• Ability to multitask and meet deadlines with minimal supervision.
• Strong time management and presentation skills as a team member.

Responsibilities

• Collect all money due at the time of service.
• Convert referrals into evaluations.
• Schedule patient visits.
• Create an inviting clinic atmosphere.
• Make all welcome calls.
• Monitor and influence arrival rate through a great customer experience.
• Manage schedule efficiently.
• Handle non-clinical documentation and deposits.
• Manage caseloads, discharge candidates, progress notes and insurance reporting.
• Monitor clinic inventory.
• Attend required trainings and complete quarterly compliance training.
