Black Hills Works, Inc. seeks a clinic nurse to deliver direct nursing care and clinic-based services to adults with physical, intellectual, and developmental disabilities in Rapid City, SD. The role includes managing clinic appointments, injections, and community-based care coordination.

The nurse will work with Direct Support Professionals, provide medical guidance, and advocate for access to community services to improve health, safety, and independence for participants.

Qualifications

• Active nursing license in SD
• Experience providing clinic-based care to adults with disabilities preferred
• Experience collecting and documenting health information accurately
• Strong communication with care teams and guardians

Responsibilities

• Schedules, confirms, and manages clinic appointments; handles walk-ins.
• Administers IM injections and oversees medication destruction per orders and policies.
• Performs Foley catheter insertion, suprapubic catheter changes, and non-delegatable wound care as required.
• Performs medical assessments and addresses emergencies as they arise.
• Maintains clinic records and documentation; inputs data into the computer system.
• Provides comprehensive case management for a small caseload; coordinates medical care and services.
• Attends specialty medical appointments as needed; documents orders and updates records.
