Job summary

Central Valley PACE in Merced, CA is seeking a Licensed Vocational Nurse for our Clinic. Under supervision, the LVN will assist in assessing and delivering clinical skilled services, collaborating with Day Center and Home Care while supporting PACE activities.

Responsibilities include patient assessment, medication administration, wound care, EMR documentation, and supervision of Patient Care Assistants. The role requires CA LVN license and current BLS CPR, with flexible hours and on-site work.

Qualifications

• Valid CA Driver’s License and reliable transportation.
• Graduate of an accredited school of professional nursing.
• Current unencumbered LVN license in CA.
• Current BLS CPR certification.

Responsibilities

• Observe participants for potential medical emergencies and provide immediate intervention within scope of practice.
• Assess patients’ physical and mental wellbeing and report findings to MD or supervising RN.
• Provide basic patient education and follow-up to implement care plans.
• Administer medications as prescribed and perform venipuncture/IV starts as ordered.
• Document medical history in the EMR and keep records current.
• Supervise Patient Care Assistants and assist Care Coordinator RN as needed.
• Review records and assist with IDT annual assessments to identify needed skilled services.
• Attend staff meetings and drive to participant homes as needed.
• Be flexible with schedule of hours worked.

Education

LVN CA license

BLS CPR

Nursing degree from accredited program
