Job summary

Habitat Health is seeking a Licensed Vocational Nurse (LVN) to support nursing care at the South LA PACE Center. You will provide direct patient care within scope, assist with care plans, and coordinate services with the interdisciplinary team to promote safety and well-being.

The role requires an LVN license, 1+ year experience with older adults, strong communication, EMR proficiency, and the ability to travel to participant homes and clinics as needed.

Qualifications

• Completion of an accredited vocational or practical nursing program; licensure required in the applicable state.
• Active, unrestricted LVN/LPN license for the state of practice.
• Minimum of 1 year nursing experience, preferably with older adults or complex needs.
• Ability to provide direct nursing care and escalate clinical concerns as needed.
• Ability to apply clinical protocols with sound judgment and emphasis on safety.

Responsibilities

• Provide direct nursing care within scope of practice and under clinical direction.
• Conduct nursing observations and assessments; escalate acute needs appropriately.
• Support care plan interventions and chronic disease management.
• Coordinate care across clinic, center, homes, and external providers.
• Educate participants and caregivers on medications, nutrition, and health needs.
• Maintain accurate clinical documentation in the EMR.
