Job summary

OptumCare is hiring a Clinic Coordinator to support central clinic operations. You will perform chart reviews, prep, and care coordination for designated regions, ensuring data accuracy across EMR and supporting the care team.

You will conduct patient outreach, manage referrals, and help schedule appointments while traveling to clinics within the region as needed. A strong customer service background is essential.

Qualifications

• High School Diploma or GED required.
• At least 1 year in health care or customer service with strong communication.
• Familiarity with healthcare terminology and EMR systems.
• Proficiency in Microsoft Office and general office procedures.

Responsibilities

• Prepare patient records in EMR before visits and review chart notes.
• Initiate patient outreach and coordinate referrals and care gaps.
• Coordinate with care teams and schedule visits with the multidisciplinary group.
• Travel to multiple work locations as needed.
