Job summary

Piedmont Healthcare, Inc. is seeking a Care Coordination Specialist to manage actionable radiology and diagnostic findings. You will oversee follow-up activities and act as the hub between providers, patients, and support services to ensure timely notifications and documentation.

The ideal candidate holds an appropriate clinical license, has hands-on experience with Epic EHR, and brings several years of care coordination exposure to a fast-paced healthcare setting.

Qualifications

• Associate's Degree in a clinical healthcare field is required.
• 3 years of clinical healthcare experience is required.
• Experience using Epic or other electronic health records is required.
• Current clinical licensure or certification in RN, ARRT, RRT, LPN, PA, or other clinically licensed professional is required.

Responsibilities

• Daily management of actionable findings work queues and coordination of follow-up activities for patients with clinically significant findings identified through radiology reports and other sources.
• Coordinate as the central point of communication among providers, patients, specialists, and support services.
• Ensure timely provider notification, patient communication, documentation, and follow-up completion.

Responsibilities

This position is responsible for daily management of actionable findings work queues and coordination of follow-up activities for patients with clinically significant findings identified through radiology reports, artificial intelligence applications, and other diagnostic sources. The coordinator serves as the operational owner of assigned findings from identification through closure, ensuring timely provider notification, patient communication, documentation, and follow-up completion. This role functions as the central point of coordination among providers, patients, specialists, and support services.

Qualifications

Education

• Associate's Degree in a clinical healthcare field Required
• Bachelor's Degree in a clinical healthcare field Preferred

Work Experience

• 3 years of clinical healthcare experience Required
• Experience using Epic or other electronic health records. Required
• Experience in radiology, pulmonary medicine, oncology, care coordination, navigation, or quality improvement. Preferred
• Experience with incidental findings programs. Preferred
• Experience working with patient outreach and follow-up workflows. Preferred

Licenses and Certifications

• Current clinical licensure or certification in one of the following: Registered Nurse (RN), Radiologic Technologist (ARRT), Respiratory Therapist (RRT), Licensed Practical Nurse (LPN), Physician Assistant (PA), or Other clinically licensed healthcare professional
