Job summary

TheKey is seeking a Client Success Manager in Clemson, SC to oversee a portfolio of aging-at-home clients, partnering with families and clinicians to design tailored service plans. You will guide caregiver teams and adjust plans to sustain high client satisfaction and retention.

The ideal candidate brings care coordination or client-management experience, strong communication, emotional intelligence, and the ability to navigate complex family dynamics in a fast-paced healthcare setting.

Qualifications

• Bachelor's Degree in Human or Health Services or related field, or equivalent work experience.
• 3 years experience in health care, elder care, social work, or related industry.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and EMR experience.
• Current driver's license and proof of insurance.

Responsibilities

• Oversee day-to-day client relationships and the delivery of high-quality, holistic care tailored to individual needs.
• Conduct assessments with prospective clients to design service plans and build rapport with families.
• Manage and supervise caregiver teams, ensuring care plans are documented, updated, and effectively implemented.
• Proactively communicate with internal and external stakeholders to evaluate solutions and ensure client satisfaction.
• Handle escalations, ensure regulatory compliance, and identify opportunities to grow the business through quality service.
• Manage the Start of Care process to ensure high-quality experiences and complete satisfaction for new clients.
• Conduct in-person re-assessments and quality assurance visits to evaluate home safety, daily routines, and evolving client needs.
