Job summary

Copper Country Community Mental Health is seeking a full‑time Client Services Manager/Supports Coordinator to serve Ontonagon County, Michigan, with travel to nearby counties as needed. The role supports individuals with serious mental illness and/or developmental disabilities, some with co-occurring substance use disorders.

Responsibilities include performing assessments, developing person-centered individual service plans, monitoring progress, and maintaining confidential records in compliance

Qualifications

• Provide Supportive Services to individuals with serious mental illness and/or intellectual/developmental disabilities.
• Conduct Assessments to evaluate needs and conditions.
• Create Individual Service Plans using a person-centered approach.
• Monitor and Evaluate progress to ensure plans are effective.
• Maintain Confidential Records in accordance with privacy standards.
