Qualifications

• Prior experience in home care or healthcare coordination preferred.
• Home Care Aide Certificate is required.

Responsibilities

• Coordinate and schedule caregivers to match client needs, preferences, and care plans.
• Serve as the primary point of contact for clients, families, and caregiving staff.

• Monitor and manage caregiver schedules to ensure consistent, uninterrupted care coverage.
• Respond promptly to scheduling changes, call-outs, and urgent client needs (may need to cover shifts).
