Qualifications

Responsibilities

• Serve as a main point of contact for clients and families.
• Handle client intake calls, onboarding, and ongoing follow-ups.
• Ensure high client satisfaction and address concerns professionally.
• Coordinate with caregivers and internal teams to support care delivery.
• Maintain accurate client documentation and records.
• Assist the supervisor with referral outreach and relationship management.
• Communicate with hospitals, clinics, physicians’ offices, and community partners.
• Help track referrals, leads, and conversion outcomes.
• Participate in community events, networking, and outreach initiatives.
• Support strategies to increase client volume and retention.
• Assist with scheduling, reporting, and internal coordination.
• Support operational workflows and process improvement.
• Provide regular updates to management on client and referral activity.
