Affirmed Home Care is seeking a Client Liaison – Pediatrics to expand our New Jersey referral network and enhance access to high-quality pediatric home care services. The role combines field work with hybrid coordination to grow partnerships across hospitals, physician groups, school districts, and community organizations.

You will leverage established relationships, develop new referral channels, and collaborate with Intake, Clinical, and Marketing teams to drive growth while ensuring

Qualifications

• Bachelor's degree in related field preferred.
• Minimum of 3 years in client liaison or related healthcare sales roles.
• Established NJ hospital/provider relationships preferred.
• Strong networking and relationship-building skills.

Responsibilities

• Develop and expand referral relationships with hospitals and clinics across New Jersey.
• Build strategic partnerships to generate new pediatric home care referrals.
• Leverage existing networks and identify new business opportunities statewide.
• Represent Affim Home Care at events and community meetings.
• Educate healthcare professionals, families, and communities about pediatric home care services.
• Collaborate with Intake, Recruiting, Clinical, and Marketing teams to support growth initiatives.
• Maintain accurate referral activity, sales tracking, and pipeline management (Salesforce).
• Meet and exceed referral growth and relationship development goals.

Location: New Jersey (Hybrid / Field-Based)

Salary: Starting at $135,000 Base Salary + Commission + Potential Signing Bonus

Help Grow New Jersey's Premier Pediatric Home Care Program

Affirmed Home Care i is expanding our Pediatrics Division throughout New Jersey and is seeking an experienced, results-driven Client Liaison to help grow our referral network and increase access to high-quality pediatric home care services.

This is an exciting opportunity for a healthcare professional with established relationships across New Jersey's healthcare and educational communities. If you have experience developing referral partnerships, growing territory revenue, and building long-term relationships with hospitals, physicians, school districts, and community organizations, we'd love to meet you.

What You'll Do

• Develop and expand referral relationships with hospitals, pediatric practices, physician groups, specialty providers, case managers, discharge planners, and school districts throughout New Jersey
• Build strategic partnerships that generate new pediatric home care referrals
• Leverage your existing professional network while identifying new business opportunities across the state
• Represent Affim Home Care at networking events, healthcare conferences, community outreach programs, provider meetings, and educational presentations
• Educate healthcare professionals, families, and community partners about our pediatric home care services and available programs
• Collaborate with our Intake, Recruiting, Clinical, and Marketing teams to support growth initiatives and ensure exceptional service
• Maintain accurate referral activity, sales tracking, and pipeline management within Salesforce and other internal systems
• Meet and exceed referral growth and relationship development goals

Qualifications

• Bachelor's degree in Business, Marketing, Healthcare Administration, Communications, or a related field preferred
• Minimum of 3 years of experience as a Client Liaison, Physician Liaison, Referral Liaison, healthcare business development professional, or healthcare sales representative
• Established relationships with New Jersey hospitals, pediatric practices, specialty physicians, school systems, case managers, and/or healthcare organizations strongly preferred
• Strong understanding of:
• NJ Medicaid
• NJ FamilyCare
• EPSDT
• IEP and 504 Plans
• Pediatric specialty care and home healthcare services
• Proven track record of generating referrals and growing provider relationships
• Excellent networking, communication, presentation, and relationship-building skills
• Self-starter with the ability to work independently while managing multiple priorities
• Valid driver's license and reliable transportation required
