Job summary

Live Well Home Care in Phoenix seeks a Client Care Specialist to coordinate client schedules, maintain accurate documentation, and ensure smooth communication between clients, caregivers, and office staff.

You will update care plans, support staffing and client retention, and handle day-to-day issues with professionalism. Prior experience in home care or healthcare administration is preferred, and strong communication skills are essential.

Qualifications

• Experience in home care, healthcare administration, or client coordination preferred.
• Strong communication and customer service skills.
• Comfortable working in a fast-paced, team-oriented environment.
• Basic computer proficiency and familiarity with scheduling software (EMR) is a plus.
• Compassionate, organized, and dependable.
• High school diploma or equivalent required; some college coursework preferred.

Responsibilities

• Act as a primary point of contact for clients and families regarding care schedules, updates, and service questions.
• Coordinate caregiver schedules and assist with shift changes, call-outs, and replacements.
• Perform regular client satisfaction check-ins to ensure high-quality service.
• Update and maintain care plans, service records, and documentation to ensure compliance with agency and state standards.
• Communicate with caregivers to provide guidance, relay updates, and ensure care continuity.
• Collaborate with the office team to support staffing, client retention, and caregiver engagement.
• Handle day-to-day issues with professionalism and attention to detail.
