Job summary

SYNERGY HomeCare Franchising, LLC is seeking a Scheduling/Client Care Coordinator to guide and coordinate all scheduling and client relationship activities. You will organize schedules for clients and caregivers, collaborate with the franchise partner and administrator, and ensure high caregiver and client satisfaction.

Some nights/weekends coverage may be needed. Ideal candidates will have 1+ year in healthcare or home care, strong computer skills (Google Suite, Excel), and familiarity with

Qualifications

• Knowledge of scheduling and on-call procedures (preferred).
• 1+ year experience in healthcare or home care (preferred).
• Proficiency with Google Suite, Excel, PowerPoint, etc.
• Familiarity with AxisCare or similar scheduling software (preferred).
• Strong leadership, organizational, and customer service skills.
• Excellent time management and written/verbal communication.

Responsibilities

• Regular communication with caregivers and clients/families.
• Onboarding clients for scheduling transfer and data integrity.
• Retention planning for client services.
• Maintain current client assessment information.
• Assist with onboarding new employees as needed.
• Conduct on-site training to ensure caregiver knowledge.
• Record home assessment information in AxisCare accurately.
• May participate in an on-call rotation.
• Submit weekly staff meeting reports.
• Assist with phones and in-service training.
• Conduct Home Assessments and follow up on client issues.
