Qualifications

• Strong documentation, organizational, and communication skills.

• Experience in substance abuse treatment or case management is a plus.

Responsibilities

• Coordinate continuing recovery services for clients returning to the community.
• Provide aftercare counseling to clients on campus.
• Perform case management for housing, job searches, and medical/social services.
• Develop referrals with community providers and schedule initial appointments.
• Collaborate with Clinical staff to ensure smooth transition from residential to continuing care.
