Job summary

WellLife Network is seeking a compassionate professional to accompany clients to medical appointments and advocate for appropriate services. You will provide direct care, assist with daily living activities, and serve as a liaison between residences and medical facilities.

You will help ensure medical appointments are completed, document outcomes in the EHR, and participate in team meetings. A valid NYS driver’s license and the ability to operate agency vehicles are required.

Qualifications

• High School diploma or equivalent required.
• Associate’s degree in Human Services preferred.
• Experience with developmentally disabled individuals or medical field experience.
• Valid NYS Driver’s license required.

Responsibilities

• Works with Residence Nurse/Residence Manager to ensure medical follow-up for all persons served.
• Accompanies persons served to medical/clinic care; advocate for needs with providers.
• Safely operate agency vans/vehicles per policy.
• Communicate immediate medical concerns to Residence Manager/Nurse.
• Participate in weekly consult review meetings with Residence Manager/Nurse.
• Ensure medical documentation and interventions are recorded in binders and/or EHR.
• Understand, document, and articulate specific medical information.
• Assist in training on ADL and self-help skills; perform total support tasks when needed.
• Work toward independence goals (budgeting, nutrition, transportation, community resources, etc.).
• Document data in the EHR system accurately.
• Manage and oversee medication per policy; proficient in EMAR data entry.
• Communicate relevant information to staff; log and report incidents.
• Physically intervene if needed and respond to dangerous situations; perform First Aid when required.
• Complete AMAP, SCIP, CPR and First Aid techniques as required.
• Maintain confidentiality at all times.
• Perform other duties as needed.
