Job Summary

We are seeking a Patient Care Coordinator to join our team. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.

We are a private pay home care company looking for a motivated, empathetic, and detail-oriented individual to become our Client Care Manager.

Responsibilities:

  1. Create and manage care schedules for our senior clients, ensuring they receive meaningful support from our caregivers.
  2. Attend client and caregiver meetings.
  3. Match caregivers to clients based on needs and skills.
  4. Schedule home visits and gather client feedback.
  5. Work with the Wellness Team to update care plans.
  6. Address client and caregiver concerns.
  7. Recognize and reward caregiver performance.
  8. Maintain positive relationships with clients and referral sources.
  9. Provide coordinated care to patients by developing, monitoring, and evaluating their home health care plans.
  10. Ensure a high level of care for the patient given by the home health aides.
  11. Schedule agency home health care aides accordingly and ensure agency aides work their scheduled shift.
  12. Manage last minute calls outs, finding appropriate coverage.
  13. Communicate with referral sources and Service Coordinators to provide an excellent customer service experience.
  14. Educate patients on their healthcare options & match them to the appropriate caregiver(s).
  15. Manage a caseload of 100+ patients.
  16. Resolve caregiver and client grievances and complaints.
  17. Address over-utilization of hours.
  18. Report personnel performance issues.
  19. Recruit and train staff.

Qualifications:

  1. High school graduate.
  2. 3 years experience in a Home Care or Service Coordination role.
  3. Demonstrated capability maintaining strict confidentiality.
  4. Proven ability to generate leads and monitor referrals.
  5. Strong typing and computer skills.
  6. Comfortable with closing/asking for business.
  7. Well organized, accurate, and attentive to detail.
  8. Excellent communication, public relations, and follow-up skills.
  9. Experience with HHA Exchange and EVV.
