Immediate need for a talented Care Manager (RN). This is a 04+ months contract opportunity with long-term potential and is located in Columbia, SC(Remote). Please review the job description below and contact me ASAP if you are interested.

Job ID: 25-75181 Pay Range: $37 - $39/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.
• Members require at least 1 phone call monthly, 1 visit every 90 days and an annual assessment.
• Case managers are allowed a of caseload of no more than 60 members due to the intensive nature of the members.
• Purpose of plan to assure members are receiving all medications, attending all MD/ mental health clinic appts, have adequate housing and transportation to attend all appts.

• Documented within 24 hours unless on Friday
• HMs get report to note who s out of compliance
