Job summary

Safe Harbor Home Care in Southern California is hiring a Client Care Coordinator to lead client assessments, update care plans, and match caregivers to client needs. You will coordinate with scheduling and operations to ensure seamless care delivery and monitor service quality and client satisfaction.

The role emphasizes caregiver development, compliance with regulations, and maintaining accurate documentation while supporting growth opportunities within Safe Harbor Home Care and The Pennant

Qualifications

• Must be 18+ with valid I-9 eligibility documents.
• California Home Care Aide registration required before client assignment.
• Current TB clearance and valid driver’s license with reliable transportation.
• Ability to maintain client confidentiality and accurate documentation.
• Ability to pass required background checks and multitask in a fast-paced environment.

Responsibilities

• Conduct client assessments and reassessments.
• Develop and update client care plans.
• Match caregivers with client needs and preferences.
• Provide caregiving coverage when necessary.
• Train, mentor, and support caregivers.
• Partner with Scheduling and Operations to maintain continuity of care.
• Monitor service quality and client satisfaction.
• Address client concerns and assist with service recovery.
• Maintain accurate client documentation and records.

What You'll Do

• Conduct client assessments and reassessments
• Develop and update client care plans
• Match caregivers with client needs and preferences
• Provide caregiving coverage when necessary
• Train, mentor, and support caregivers
• Coach and support caregiver development
• Partner with Scheduling and Operations teams to maintain continuity of care
• Monitor service quality and client satisfaction
• Address client concerns and assist with service recovery
• Support caregiver accountability and performance improvement
• Maintain accurate client documentation and records
• Monitor compliance with company policies and regulatory requirements
• Communicate effectively with clients, families, caregivers, and office staff
• Assist with onboarding activities and caregiver support initiatives
• Support quality assurance efforts and follow-up activities

Qualifications

• Must be at least 18 years of age
• Valid I-9 employment eligibility documentation
• California Home Care Aide (HCA) registration prior to client assignment
• Current TB clearance
• Valid driver's license and reliable transportation
• Ability to maintain accurate documentation and client confidentiality
• Ability to pass required background check
• Strong organizational and prioritization skills
• Strong written and verbal communication skills
• Leadership, coaching, and problem-solving abilities
• Ability to manage multiple priorities in a fast-paced environment
• Commitment to client satisfaction and quality care

Preferred Qualifications

• Previous experience conducting client assessments
• Prior caregiving leadership experience
• CNA Certification
• HHA Certification
• Experience with WellSky or similar software
• Client relations, service coordination, or case management experience
• Bilingual (Spanish/English or Tagalog/English)
