Job summary

Safe Harbor Home Care is seeking a Client Care Coordinator in California to lead client assessments, care planning, and caregiver matching. You will collaborate with scheduling and operations to ensure continuity of care and high client satisfaction.

The role emphasizes leadership, coaching, and robust communication to support caregivers and clients in a fast-paced in-home care setting. Growth opportunities exist within the company.

Qualifications

• Must be 18+ years old.
• California HCA registration prior to client assignment.
• TB clearance required.
• Valid driver’s license and reliable transportation.
• Ability to maintain client confidentiality and accurate documentation.
• Background check clearance.
• Strong organizational and prioritization skills.
• Excellent written and verbal communication.
• Leadership and coaching abilities.
• Ability to manage multiple priorities in a fast-paced environment.
• Commitment to client satisfaction and quality care.

Responsibilities

• Conduct client assessments and reassessments.
• Develop and update client care plans.
• Match caregivers with client needs and preferences.
• Provide caregiving coverage when necessary.
• Train, mentor, and support caregivers.
• Coach and support caregiver development.
• Partner with Scheduling and Operations to maintain continuity of care.
• Monitor service quality and client satisfaction.
• Address client concerns and assist with service recovery.
• Support caregiver accountability and performance improvement.
• Maintain accurate client documentation and records.
• Ensure compliance with company policies and regulatory requirements.
• Communicate effectively with clients, families, caregivers, and staff.
• Assist with onboarding activities and caregiver support initiatives.
• Support quality assurance and follow-up activities.

What You'll Do

Qualifications

• Must be at least 18 years of age
• Valid I-9 employment eligibility documentation
• California Home Care Aide (HCA) registration prior to client assignment
• Current TB clearance
• Valid driver's license and reliable transportation
• Ability to maintain accurate documentation and client confidentiality
• Ability to pass required background check
• Strong organizational and prioritization skills
• Strong written and verbal communication skills
• Leadership, coaching, and problem-solving abilities
• Ability to manage multiple priorities in a fast-paced environment
• Commitment to client satisfaction and quality care

Preferred Qualifications

• Previous experience conducting client assessments
• Prior caregiving leadership experience
• CNA Certification
• HHA Certification
• Experience with WellSky or similar software
• Client relations, service coordination, or case management experience
• Bilingual (Spanish/English or Tagalog/English)
