Job summary

Safe Harbor Home Care is seeking a Client Care Coordinator to lead caregiving teams, develop and update client care plans, and ensure high-quality, compliant service. You will match caregivers to client needs, monitor satisfaction, and support operations to maintain continuity of care.

The role emphasizes staff development, effective communication with families, and ongoing training. A strong focus on client satisfaction and quality care is essential.

Qualifications

• Must be at least 18 years old.
• California HCA registration required prior to client assignment.
• Current TB clearance.
• Valid driver’s license and reliable transportation.
• Ability to maintain client confidentiality and accurate documentation.
• Pass background check.
• Strong written and verbal communication skills.
• Excellent organizational and prioritization abilities.

Responsibilities

• Conduct client assessments and reassessments.
• Develop and update client care plans.
• Match caregivers with client needs and preferences.
• Provide caregiving coverage when necessary.
• Train, mentor, and support caregivers.
• Coach and support caregiver development.
• Coordinate with Scheduling and Operations to maintain continuity of care.
• Monitor service quality and client satisfaction.
• Address client concerns and assist with service recovery.
• Support caregiver accountability and performance improvement.
• Maintain accurate client documentation and records.
• Ensure compliance with company policies and regulatory requirements.
• Communicate effectively with clients, families, caregivers, and office staff.
• Assist with onboarding activities and caregiver support initiatives.
• Support quality assurance and follow-up activities.

What You'll Do

Qualifications

• Must be at least 18 years of age
• Valid I-9 employment eligibility documentation
• California Home Care Aide (HCA) registration prior to client assignment
• Current TB clearance
• Valid driver's license and reliable transportation
• Ability to maintain accurate documentation and client confidentiality
• Ability to pass required background check
• Strong organizational and prioritization skills
• Strong written and verbal communication skills
• Leadership, coaching, and problem-solving abilities
• Ability to manage multiple priorities in a fast-paced environment
• Commitment to client satisfaction and quality care

Preferred Qualifications

• Previous experience conducting client assessments
• Prior caregiving leadership experience
• CNA Certification
• HHA Certification
• Experience with WellSky or similar software
• Client relations, service coordination, or case management experience
• Bilingual (Spanish/English or Tagalog/English)
