CBI is an integrated behavioral healthcare agency providing residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness across Arizona.
Client Care Coordinator – Responsibilities
- Manage housing caseloads and coordinate care for individuals and families experiencing homelessness.
- Coordinate and monitor RBHA and non‑RBHA community‑based services, facilitating delivery and ensuring connections to PCMH, PCP, or new PCMH, PCP services.
- Coordinate through CBI Continuum of Care or other community agencies, addressing social determinants of health via housing and community integration programs.
- Identify housing units using housing search software systems.
- Complete data entry into established systems to support program outputs and outcomes.
- Assist higher‑level personnel and leadership with ongoing reports.
- Produce end‑of‑shift reports documenting coordination of care completed for each individual.
- Work as part of a multidisciplinary team, coordinating with nursing, counseling, and clinical staff, and document interactions per Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, and CBI policies.
Qualifications & Requirements
- High school diploma or GED required. Associate degree in a field related to Behavioral Health preferred.
- Minimum of twelve months of recovery from substance use and/or mental health disorders required.
- Minimum of six months of paid related work experience with exposure to both behavioral and medical crisis situations required.
- 1–3 years of full‑time Health Care related work experience preferred.
- Valid identification required.
- Will complete CBI Peer Certification within 90 days of being hired.
- Arizona Fingerprint Clearance Card (Program Specific).
