Comfort Keepers is seeking an outgoing External Client Care Coordinator in Allentown to build trust with families and guide them through home care options. You will establish relationships with providers, visit clients, and coordinate services with caregivers to ensure a positive experience.

Responsibilities

• Connect with individuals and families to discuss home care needs and guide them through services.
• Establish and maintain relationships with healthcare providers and referral partners.
• Visit clients to ensure outstanding care and address questions or concerns.
• Coordinate services with caregivers and office staff to ensure a positive client experience.
• Support onboarding of new clients and smooth transition into care.
• Participate in community outreach and represent Comfort Keepers at events.
• Keep client information organized and maintain accurate records.
• Communicate with families and care team to ensure quality service.
