In this dynamic role, you will play a critical part in connecting patients with timely, high-quality home health care while collaborating with clinical staff and community agency partners in an engaging, professional office environment.

Key Responsibilities:

• Review, validate, and process Care at Home referrals for outsourced community agency placement.
• Update referral statuses, episode details, and authorizations in Epic and electronic medical records.
• Research and troubleshoot referral escalations, exceptions, and Letters of Agreement (LOA).
• Coordinate with intake teams, nurses, and external partners to clarify needs and resolve discrepancies.
• Monitor assigned workqueues to maintain productivity, timeliness, and strict HIPAA compliance.

Qualifications & Requirements:

• High school diploma or GED required (Bachelor's degree preferred).
• 2+ years of experience in healthcare operations, referral coordination, or authorization processing.
• Experience with EMR systems and workqueues (Epic experience strongly preferred).
• Knowledge of healthcare insurance, payer coverage rules, and authorization workflows.
