Part‑time Registered Nurse Case Manager – Oakland AreaAnticipated Territory: Oakland Area
Schedule: Must be available full days every Saturday and Sunday (4 SOCs/day). Also willing to consider full time: Saturday and Sunday plus 3 weekdays.
What you’ll do- Conduct Start of Care (SOC) admissions – OASIS
- Conduct follow‑ups and evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to the clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Concierge for case managers: Dedicated resources assist with time‑consuming administrative tasks
- Flexible work schedules: Create your own schedule and hours
- Targeted territories: Typically patient visits are within a 5‑15 mile radius
- Autonomy: No clocking in/out

- Previous experience with IV medication infusions is highly preferred
