AbsoluteCare Medical Center & Pharmacy seeks a Provider Relations Manager – CKD to lead field-based partnerships with nephrology practices, primary care groups, and dialysis providers across Ohio. Build, educate, and expand the CKD program while aligning with value-based care and care coordination goals.

You will travel up to 50% within Ohio, manage relationships, and translate clinical insights into provider-focused strategies to improve CKD outcomes and program adoption.

Qualifications

• Bachelor’s degree or equivalent in a healthcare/business field.
• 5+ years in provider relations, healthcare sales, or account management.
• Proficient with CRM systems and data tools; ability to translate data to providers.
• Willingness to travel up to 50% within Ohio.

Responsibilities

• Develop and deepen CKD referral partnerships across Ohio.
• Educate providers on AbsoluteCare CKD care model and value-based partnerships.
• Manage field-based accounts with proactive planning and follow-ups.
• Collaborate with analytics and operations to align provider engagement with outcomes.
