Job summary

Aunt Martha’s Health and Wellness is seeking an Intake Coordinator to review and assess case eligibility for CIPP/DCIPP meetings. You will coordinate case reviews, scheduling, and documentation while working with internal teams and external partners to improve service delivery for children and families.

Responsibilities include gathering critical information, entering referrals, scheduling meetings, and maintaining accurate case records in our electronic system.

Qualifications

  • High School Diploma and at least two years of experience in casework or a related field.
  • Bachelor’s Degree and two years of professional casework/case management experience preferred.
  • Experience working with Clinical Intervention for Placement Preservation and DCFS preferred.
  • Strong organizational, communication, and computer skills.

Responsibilities

  • Receive case notifications and provide CIPP/DCIPP Universal Referral Forms to Caseworkers for completion.
  • Review and assess completed paperwork to determine case eligibility for CIPP/DCIPP meetings.
  • Research and gather critical case information including Child Protection reports, medical records, school reports, psychological evaluations, caregiver data, and court documents.
  • Enter referrals into the CIPP/DCIPP scheduling system and distribute referral materials to facilitators.
  • Coordinate and schedule CIPP/DCIPP meetings with identified participants such as biological parents, children/youth, substitute caregivers, DCFS/POS Supervisors, Caseworkers, and Guardians Ad Litem.
  • Provide caseworkers and supervisors with necessary documentation for CIPP/DCIPP meetings.
  • Build and maintain effective working relationships with internal teams, caseworkers, supervisors, and external partners.
  • Attend and participate in various regional meetings as needed.
  • Assist in quality assurance and improvement efforts related to the CIPP/DCIPP process.
  • Support case triage and emergency review planning with supervisors.
  • Maintain accurate case records within the organization’s electronic system, ensuring compliance with all required guidelines.
  • Generate status and administrative reports for internal teams and leadership.

Position Summary

The Intake Coordinator is responsible for reviewing, assessing, and evaluating case eligibility for CIPP/DCIPP meetings.
