Lawndale Christian Health Center is seeking a Chronic Disease Coordinator to coordinate patient support services and education for chronic disease management at the Health Center. This role reports to the Population Health Department and supports both on-site and remote monitoring programs.

• Comfortable in either small group or large group settings.
• Strong ability to independently multi-task and work well in a fast-paced, team environment.
• Demonstrates problem-solving skills and able to follow protocols and established processes.
• Able to be detail-oriented, think critically, troubleshoot, analyze processes, and develop corrective actions for process improvement.
• Strong organizational and time management skills.
• Computer literacy and experience is required including proficiency in Microsoft Office Suite.
• Experience working with electronic medical records.
• Able to work a flexible schedule, including evenings.
• Must possess a valid driver’s license

Position Summary:

The Chronic Disease Coordinator has a key role on the Chronic Disease Team with the responsibility of coordinating patient support services and patient education for both on-site and remote monitoring programs that support chronic disease management at Lawndale Christian Health Center. This position will report to the Population Health Department.

Required Knowledge, Experience, or Licensure:

• Client Services Experience recommended.
• Bilingual in English and Spanish is a plus.
