Responsibilities

• Coordinate care for patients with chronic diseases to improve health outcomes and engagement in preventive care services.
• Identify patients with care gaps, missed follow-ups, or unmanaged chronic conditions and facilitate outreach and care coordination.
• Support chronic disease management workflows, preventive care initiatives, and transitions of care.
• Contribute to safe and efficient provision of care across neonatal, infant, adolescent, and adult populations.
