The role involves identifying care gaps, outreach, and care coordination within primary care practice settings, with a focus on safe, efficient care across neonatal to adult populations. Reports to the Director of Medical Practices.

Formación

• Education level equivalent to completion of a Licensed Practical Nurse program.
• Knowledge of chronic disease management.
• Current license to practice professional nursing in Georgia; BLS Provider certification required.
• Three to five years clinical experience in primary care.

Responsabilidades

• Coordinate care for patients with chronic diseases to improve health outcomes and engagement in preventive care services.
• Identify patients with care gaps, missed follow-ups, or unmanaged chronic conditions and facilitate outreach and care coordination.
• Support chronic disease management workflows, preventive care initiatives, and transitions of care.
• Contribute to safe and efficient provision of care across neonatal, infant, adolescent, and adult populations.
