Qualifications

• One year of related experience.
• Georgia Licensed Practical Nurse or Multistate Nursing license.
• Proficient at computer programs such as Word, Meditech and Epic.

Responsibilities

• Coordinate a wide range of self-management support and disease registry activities for the clinic population.
• Collaborate with physicians, staff and other health care professionals to ensure safety and high-quality care.
• Work with the PCMH Development Team and quality committees to improve population health outcomes.
• Document care coordination activities and use Health IT systems to support patient care.
