Hcshiring is seeking a Licensed Practical Nurse (LPN) to join our community health team in Duluth, Georgia. The role emphasizes in-home visits, coordinating services, and maintaining accurate member records to support high-quality care.

Ideal candidates have an active LPN license with at least one year of relevant experience, strong communication and organizational skills, and the ability to work closely with members, caregivers, and primary care physicians.

Competenze

• Valid LPN license with at least one year of experience in health and human services.
• Experience in social work, home and community based services, healthcare or geriatrics preferred.
• Good communication and customer service skills.
• Excellent organizational and decision making skills.

Mansioni

• Identify, plan, coordinate community based services for members.

• Conduct in-home visits and assessments.
