Job summary

Summit Spine and Joint Centers in Georgia is seeking a full-time Licensed Practical Nurse (LPN) to join our Chronic Care Management team. The role supports patient outreach, follow-up, and detailed documentation within the EMR to coordinate ongoing care.

The ideal candidate is organized, tech-savvy, and communicates well with patients, with strong written skills and a collaborative approach with RNs and providers. A hybrid/remote schedule is available.

Qualifications

• Current and unrestricted LPN license.
• Minimum of one year of professional LPN experience.
• Strong verbal and written communication skills.
• Comfortable learning and utilizing EMR technology.
• Ability to manage multiple patient interactions and documentation.

Responsibilities

• Conduct patient outreach and follow-up as part of CCM.
• Communicate with patients regarding ongoing care needs and appointments.
• Support care coordination for patients with chronic conditions.
• Document patient interactions and care in the EMR accurately.
• Maintain timely and accurate patient records and HIPAA compliance.
• Collaborate with RNs, providers, and CCM team members.
