Job summary

Summit Spine and Joint Centers is seeking an experienced full-time Registered Nurse to lead its Chronic Care Management team. The role focuses on coordinating care for patients with chronic conditions and providing clinical guidance within the CCM program.

The ideal candidate will have 5+ years of nursing experience, strong communication, and proven leadership in high-pressure settings. EMR proficiency and HIPAA compliance are essential for this position.

Qualifications

• Current and unrestricted Registered Nurse (RN) license.
• Minimum of five (5) years of professional nursing experience.
• Clinical experience in ICU, Med-Surg, ER, or comparable acute-care environment.
• Strong clinical assessment and critical-thinking skills.
• Excellent verbal and written communication skills.
• Strong organizational skills and attention to detail.

Responsibilities

• Provide clinical leadership and support to the Chronic Care Management team.
• Coordinate and manage care for patients enrolled in the CCM program.
• Communicate with patients regarding care plans, medications, appointments, and other needs.
• Review patient information and identify concerns requiring clinical intervention or escalation.
• Provide nursing guidance within RN scope of practice and assist with complex patient situations.
• Coordinate with providers and healthcare team to support continuity of care.
• Maintain accurate documentation within the EMR and ensure HIPAA compliance.
