Responsibilities

• Enter new patient demographics and prepare kits for delivery.
• Schedule initial home visit, appointments and equipment delivery.
• Maintain census and retrieve member information from portals.
• Schedule Interdisciplinary Care Team meetings and record minutes.
• Respond to inquiries from patients, care managers and physicians.
